You have to have a lot of healing items and a lot of patience basically.

You'd have to rely on a lot of things, like Aeons, Rikku's Mix and Use, and passive abilities in weapons (slow, poison, etc).

What about solo battles? Are they just for bragging rights?

All the challenges are for bragging rights, but if you complete a NSGNSNCNONENNENBB challenge (no sphere grid, no summons, no customising, no overdrives, no escape, no No Encounters, no blitzball), you definitely have something to be proud of. As far as I know BFA has never been defeated under those conditions.

Eh, not surprising? You'd basically get killed anytime you encountered an enemy. This challenge would be known as "Gil talks", as you would have to rely on weapons and armour you can buy. Or find, for that matter.

It doesn't really seem possible, but I guess it could be done. I don't think I would have the patience to do it. Imagine trying to battle Nemesis or Penance with that handicap.

Or make it even harder: all the things above PLUS use the equipment you obtained in the beginning only.
I'd actually like to give it a try. Not the hardest possible, I'd just use the start weaponry, but do everything else normally (SG, Overdrives, Aeons etc.). Auron's Katana looks so wickedly cool. :)

May not be the hardest possible, but it's gotta be pretty close. If I had to go with the starter weapons, I would at least let Tidus use the Brotherhood.

If you allow the customisation of the initial equipment, it's not too difficult. First Strike and SOS Haste are all you need. Tidus is automatically equipped with the Brotherhood, so that makes things a little easier, unless you're against a Water fiend :)
