Skate Heaven; go straight from the beginning of the level onto the huge QP wall. Grind the entire rail on top of the wall & keep going. You will get the BTT "Van Secret Area Key". A new level opens near the wall (I think that's it; I think there are a couple of hidden areas, I don't remember which is which).

In Skate Heaven you Grind the gutter on a building and jump into the volcano, getting the "Feed the Volcano" gap or something. Then you'll end up at the half-pipe.
